Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Without defining the pr_fmt() macro, the "tape: " prefix will not be
printed when using the pr_xxx printk macros. This patch adds the
missing definitions.

With CONFIG_DEBUG_OBJECTS_TIMERS=y "chccwdev --online" for a tape device
will fail with message "ODEBUG: object is on stack, but not annotated".
We now use init_timer_on_stack.

In case of error, functions dasd_kmalloc_request and idal_buffer_alloc
return an ERR pointer, but never return the NULL pointer. So after a
call to one of these functions, a NULL test should be replaced by an
IS_ERR test.
A simplified version of the semantic patch that makes this change is

After switching compression on/off with the mt command, tape encryption is no
longer working. The reason for that is, that the modeset_byte is set to
the compression value instead of using bitwise and/or bit operations to
enable/disable the corresponding bit.

If a tape device is assigned to another host, the interrupt for the assign
operation comes back with deferred condition code 1. Under some conditions
this can lead to an endless loop of retries. Check if the current request is
still in IO in deferred condition code handling and prevent retries when the
request has already been cancelled.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
