Minor fixes that weren't urgent enough to go into 3.3. The two paz00
patches are in mainline already but got merged after the branch point
for this branch, so they're duplicated. I also ended up merging in
rmk/for-armsoc into this later on to fix a bug introduced by it, so it's
included but it's not the base of this branch.

The mach/dm365.h and mach/dm646xh headers are currently being included
in some device drivers. The removal of those includes is already merged
in some subsystem trees, but adding the dummy files ensures that this
branch can be built standalone.

There are instances of IO_ADDRESS() being used for system module
(sysmod) register access. Eliminate this in favor of a ioremap()
based access. ioremap() the entire sysmod address space once during
boot-up and provide a helper macro to access specific register
offsets within the address space.
With this, also eliminate ioremap() of specific sysmodule registers
related to VPIF happening in DM646x EVM code.
While at it, also eliminate some duplicate sysmod register offset macros
defined in code and place offset definitions at one place in davinci.h

Remove individual platform header files for dm365, dm355, dm644x
and dm646x and consolidate it into a single and common
header file davinci.h placed in arch/arm/mach-davinci.
This reduces the pollution in the include/mach and is consistent
with Russell's suggestions as part of his "pet peaves" mail.

While at it, fix the forward declaration of spi_board_info,
and include the right header file instead.
The further patches in the series take advantage of this consolidation

Drivers should no longer use omap_read/write functions
but instead use ioremap + read/write functions.
As some USB legacy code is still shared between omap1 and
omap2420, let's limit the omap_read/write to plat/usb.h.
Note that the long term fix is to update the drivers to
use ioremap and read/write functions. That can now be
done as a separate patch series that is limited to the
USB drivers.
Also make sure the legacy omap1-keypad.c driver builds
if selected for 2420 based systems.

Remove omap_{read,write}l() from the 24xx PM code. The clocksource
code should now handle what this was supposed to do.
Tested on N800 -- but it's hard to say whether this fixes anything.
OMAP24xx static suspend path is currently broken, and this patch
doesn't change that.

Fix the bit field width information for the IPSR4 register
in the r8a7779 pin function controller (PFC).
Without this fix the Marzen board fails to receive data
over the serial console due to misconfigured pin function

We have a few problems when returning to userspace. This is a
quick set of fixes for 3.3, I'll look into a more comprehensive
rework for 3.4. This fixes:
- We kept interrupts soft-disabled when schedule'ing or calling
do_signal when returning to userspace as a result of a hardware
interrupt.
- Rename do_signal to do_notify_resume like all other archs (and
do_signal_pending back to do_signal, which it was before Roland
changed it).
- Add the missing call to key_replace_session_keyring() to
do_notify_resume().

In commit 54321242af ("Disable interrupts early in Program Check"), we
switched from enabling to disabling interrupts in program_check_common.
Whereas ENABLE_INTS leaves r3 untouched, if lockdep is enabled DISABLE_INTS
calls into lockdep code and will clobber r3. That means we pass a bogus
struct pt_regs* into program_check_exception() and all hell breaks loose.
So load our regs pointer into r3 after we call DISABLE_INTS.
