5d89a48acf altera_uart: Fix missing prototype for registering an early console 
Simply add an early_altera_uart_setup() prototype declaration, otherwise
platform code have to do it in .c files, which is ugly.

0d426eda7c altera_uart: Add support for different address strides 
Some controllers implement registers with a stride, to support
those we must implement the proper IO accessors.
