pci_fixup_irqs() used to call pcibios_map_irq on every PCI device, which
for RT2880 included bus 0 slot 0. After pci_fixup_irqs() got removed,
only slots/funcs with devices attached would be called. While arguably
the right thing, that left no chance for this driver to ever initialize
slot 0, effectively bricking PCI and USB on RT2880 devices such as the
Belkin F5D8235-4 v1.
Slot 0 configuration needs to happen after PCI bus enumeration, but
before any device at slot 0x11 (func 0 or 1) is talked to. That was
determined empirically by testing on a Belkin F5D8235-4 v1 device. A
minimal BAR 0 config write followed by read, then setting slot 0
PCI_COMMAND to MASTER | IO | MEMORY is all that seems to be required for
proper functionality.
Tested by ensuring that full- and high-speed USB devices get enumerated
on the Belkin F5D8235-4 v1 (with an out of tree DTS file from OpenWrt).

days, so remove the additional ioremap_nocache interface.

Drop the __init from pcibios_map_irq() to make this section mis-
match go away:

The function pcibios_scanbus() references
the function __init pcibios_map_irq().
This is often because pcibios_scanbus lacks a __init

Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends. That changed
when we forked out support for the latter into the export.h file.
This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig. The advantage
in doing so is that module.h itself sources about 15 other headers;
adding significantly to what we feed cpp, and it can obscure what
headers we are effectively using.
Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each obj-y/bool instance
for the presence of either and replace as needed.
We also needed to remove the no-op MODULE_DEVICE_TABLE usage in
several instances to permit removal of the module.h include. The
files in these instances were all controlled by bool Kconfig.
In one instance, module_param was being used so we transition the
module.h include onto a moduleparam.h include.

c861519fcf ("MIPS: Fix delay loops which may
be removed by GCC.") which made it upstream was an outdated version of the
patch and is lacking some the removal of two variables that became unused
thus resulting in further warnings and build breakage. The commit
from ae878615d7cee5d7346946cf1ae1b60e427013c2 was correct however.

GCC 4.1 and newer remove empty loops. This becomes a problem when delay
loops get removed. Fixed by rewriting to user the proper Linux interface
for such delays.
