The IS_ENABLED() macro checks if a Kconfig symbol has been enabled either
built-in or as a module, use that macro instead of open coding the same.
Using the macro makes the code more readable by helping abstract away some
of the Kconfig built-in and module enable details.

With the omap1 SPARSE_IRQ changes mach/irqs.h is no longer
automatically included. Turns out now we rely on ARCH_OMAP15XX
including hardware.h from memory.h, so without ARCH_OMAP15XX
we get build failures.
As we have legacy drivers still relying on these indirect
includes, let's not add more mach includes to the drivers.
Those have to be removed anyways for multiplatform support.
Let's fix up mach-omap1 to include soc.h where cpu_is_omap
checks are done, and common.h for board-*.c files.
But let's keep the indirect memory.h include for now to avoid
unnecessary churn in the drivers.

We need to remove this from plat for ARM common zImage
support.
Also remove includes not needed by the omap_hsmmc.c driver.

DMAengine uses the DMA engine device structure when mapping/unmapping
memory for DMA, so the MMC devices do not need their DMA masks
initialized (this reflects hardware: the MMC device is not the device
doing DMA.)

Convert arch/arm includes of mach/gpio.h and asm/gpio.h to linux/gpio.h
before we start consolidating the individual platform implementations
of the gpio header files.

As noted by Michał Mirosław <mirqus@gmail.com>, the voltages should
cover the supported voltage range, or support only one voltage.
As all these boards are using a GPIO to enable the power, chances
are that only 3.3V cards are supported on these boards.

This will simplify the MMC low-level init, and make it more
flexible to add support for a newer MMC controller in the
following patches.
The patch rearranges platform data and gets rid of slot vs
controller confusion in the old data structures. Also fix
device id numbering in the clock code.
Some code snippets are based on an earlier patch by
Russell King <linux@arm.linux.org.uk>.

Most of the omap1 MMC boards got broken by an earlier patch
138ab9f832. If you look closely,
the MMC init funtions are pretty much just stubs.
Remove broken init code to make room for cleaner MMC init code.
