KVM/nVMX: Use kvm_vcpu_map for accessing the shadow VMCS

Use kvm_vcpu_map for accessing the shadow VMCS since using
kvm_vcpu_gpa_to_page() and kmap() will only work for guest memory that has
a "struct page".
